every few days or so a random planet appears on my planet list however, when I click on the planet, I cannot access the planet screen, I checked my ships log or any planetary alerts and I usually find other planet's names that were alerted, but not the random ones that appear in my list.

We had the same issue here in Paladin's, every few days a 5x colossal plasma would appear on our planet tabs, enemy occupied. I sent a ticket to Dan and he confirmed its a glitch that sometimes occurs, anyway he removed it from our scans and not had the issue since.
