Deep Meditation
Go 30 days without leaving a legion, sending an alert on a planet, and/or sending an alert on an enemy player.
Does this mean I have to go 30 days without doing ANY of these actions, or I just have to go 30 days without doing ONE of the actions?

Must be all of them. I alerted a planet about 12 hours before achievements arrived and started on 0/30 (and I have never left the legion), but someone I know (who hadn't alerted or left the legion) was awarded the temple immediately.
If I had to guess, the "and/or" applies to only Planet and Player alerts, as in "you can't alert a planet and/or a player, in addition to not leaving the legion".
